I'm wondering if there might be a way to enclose the candle flame part somehow so that it looks like an old fashioned flicker flame light bulb. I can trim down the part with a Stanley knife and put it in a clear minifigure head, but that is not quite in the spirit of Lego. Solution must be no wider than a minifigure head.

As far as I know there is not a single part that fits that flame and is smaller than a minifig head. The smallest that might fit the flame is the globe around the lamp of the Sesame Street set released this year.

The easiest way for if you plan to keep the flame upwards you could use these pieces:

https://www.bricklink.com/v2/catalog/catalogitem.page?P=4740#T=C as base, with stud downwards

https://www.bricklink.com/v2/catalog/catalogitem.page?P=20482#T=C through the hole of 4740 from the bottom

Flame through the little hole of 20482 that sticks through 4740

And finally the round globe on top of it, so it rests on 4740.

A bit like the flames in https://www.bricklink.com/v2/catalog/catalogitem.page?S=70608-1&name=Master Falls&category=[The LEGO Ninjago Movie]#T=S&O={"iconly":0}, but with a globe on top :P

 

The harder way: there is a little hole at the top of the globe, which fits the flame. So if you have reeeaaaaly small fingers or a pincer you might be able to get the flame through that hole from the inside. Dunno how you would fill in the bigger hole :P

The part in Sesame Street isn't new, just new in that "opal" color, and the clear versions aren't too pricey having been in a bunch of batman sets.  That might be your best bet if that specific size requirement is set in stone.  Next best thing (and easier to work with) might be the 2x2 dome (part 30151).  You could easily put the flame in 20482 and that between 4 studs that the 30151 is affixed to.
