What happened to "The Lego Adventure Book Volume 4"?

I was wondering what happened to "The Lego Adventure Book Volume 4", it was suppose to be published in late 2016, but I have never been able to buy it.

No-Starch (the publisher) never put it on their website, even though they have volumes 1, 2, and 3 advertised way before publishing. I only found out about it because I was expecting it (I already had the other volumes).

It might be because of the passing away of Daniel August Krentz (1937-2016) who was a very good friend of Mark and Meg.  you can read about Daniel and his amazing work here.

Maybe, but it doesn't explain why they made a cover for volume 4, and that there are some websites that have it listed...

Yup, last I saw it was due out Spring 2017 according to the publisher.  No Starch then decided not to go through with it, at this time.  Might not be what you want to hear, but that's the answer I got from the company.
